As a new patient, we want to ensure your first appointment runs as smoothly as possible. If you are a new patient with a scheduled appointment please allow 30 minutes prior to your appointment to fill out introductory forms. For your convenience, you may also print out a patient registration form to complete and bring to your appointment. If you chose the second alternative you need to arrive only 10 minutes early.

A Balanced Life Health Care accepts payment by cash or personal check. Patients are responsible for understanding their insurance plan and preparing for any payments necessary at the time of service.
